Being unaware of appropriate and available resources heightens this frustration.

Meet Our Management Team
Rachel E. Webb, Owner/Administrator
Rachel E. Webb founded Extended Family Home Care in January 2007. She has a degree in Social Work and has been providing Home Health Care and Geriatric Care Management Services for over fifteen years. Rachel is originally from Minneapolis and attended the University of Kansas prior to settling in Denver in 2002. The Rocky Mountains and the 300+ days of sunshine claim drew Rachel to Colorado. Her desire was to establish a premier home care agency to best meet the needs of Colorado’s aging population. Rachel realized the need for an agency that would provide the quality of care she would require for her own family.

Rachel’s favorite part of her job is the time spent with clients. She enjoys hearing their stories and making them feel as special as they are. Rachel continues to love calling Colorado “home” after thirteen years and lives with her husband, Jon, and their two boys who bring her great joy. Rachel directly oversees the daily operations of EFHC. She is personally involved with our clients, caregivers and management staff. “We are very grateful to be a trusted resource for Colorado seniors and their families.”
Leah Diaz, Care Coordinator
Leah’s career in Home Health Care began after her decision to leave the corporate world. Leah earned her CNA license and found her place comfortably among the staff of Extended Family Home Care, earning a promotion as a Caregiver Supervisor and then Care Coordinator.
Leah is very involved in the daily communications with clients, their families, and our care team and management staff. “It is very rewarding to work for a company that truly values both their clients and their employees. I really enjoy being involved with the day-to-day interests of our clientele and caregiver team, making sure that each client receives exceptional care.” Leah brings extensive experience in both client care and human resources to our team. Her attention to detail and quick, accurate response to phone calls and emails is something that our clients, client’s families, and care team have come to depend upon. Leah is delightful and her positive attitude and personal integrity are very much appreciated.

Jon Webb, Quality Assurance Manager
Jon is the Quality Assurance Manager at Extended Family Home Care. He ensures the agency continues to be compliant with the licensing requirements of the Colorado Department of Public Health & Environment. Jon helps run a very successful Quality Management Program at EFHC and strongly encourages involvement from clientele and staff. He also oversees the general administrative aspects of the business including invoicing and payroll, bookkeeping, and financial reports. Jon assists our clients with their long term care insurance policies.
Jon’s mother, Jackie, spent her lifetime in Social Work and Nursing Home Administration before co-founding EFHC with Rachel. Jon has always taken a special interest in the residents, and has had a passion to help seniors grow old with dignity. “I feel privileged to help our elders maintain the highest quality of life for as long as possible in their home setting.” A lifelong resident of Colorado, Jon enjoys the outdoors and is an avid Tournament Bass Fisherman. “We look forward to caring for you, your family, and thank you for placing your trust in us.”
